Images and files are missing

When you view a tutorial in the publishing section there is an image missing:
And the uploaded images and files are missing too
And another problem when you write a tutorial and want to choose a category it appears the name of the image instead of name of the category for example: instead of PHP there is and icon_tutorial_php.gif
