i'm not sure a tutorial for this would be quite useful as its only an image with some text on it. Just take a picture of a real one or there many on internet already and put some texts on it with a GFX editor... (even Paint could do the job)

Thats with PHP and possibly using a flat file database instead of mysql. It is a pretty nice script, I might right a tutorial on it when I have the time and figure it out myself.
