Form In Flash? Help

Ok guys, I need to create a form in flash that allows the user to choose from check boxes for things like title, so the check boxes would have MR, MRS, MSTR,MS, etc. and drop down lists for Date of birth, ive been doing flash for a few months but this is the first time i have tried anything like this its all going horribly wrong every tutorial i have looked at doesn't seem to work.

I will then need flash to preferably take the data inputted and place it into a microsoft access database or e-mail the information to the supplied email address so it can be manually entered to the database.

Could you guys give me some pointers, i am really stuck on this i don't know how to do checkboxes, drop down lists and the fancy stuff. If it isn't possible in flash, could you create a HTML page that displays inside of flash? or just link off the site to a html sign up page? if so ive got to sort out how to do a sign-up page in HTML.

Open the components panel :blink: There you have checkboxes, radiobuttons, dropdownboxes and much more..

Sending those to a db or a email, there is a tutorial over at http://www.communitymx.com or at http://www.flash-db.com which explains it pretty well

If your going to mail it, you need to use php for that.

I think he's on a MS environment.. never seen a hoster using ms access with php :)
