Help with Form Techniques

I am making a form. And I have a number of fields for the user to fill out. I don't want them to feel obligated to have to fill each one out. So I want each input field on viewable after the user clicks a check box to the left of the input's title. I hope that was clear...for example....



If I had a Form asking you for your Name, you would only be able to see the text field to type in AFTER!!! you click a check box to the left of the label "Name".



I'm pretty sure I've seen this before, and I've never thought to look to see how it's done, I'm thinking its Java? If anyone can help me out with this, it would be awesome.

Thank you so much. I actually found a page in the p2l tuts that i was reading when I saw you posted. Between the two, I feel im in okay shape.
