Create a Birthday Card

I would like to create a birthday card for a friend of mine from scratch. I was thinking either photoshop or illustrator would be the place to do something like that, but I am not too sure how to get started. What dimensions I should use or anything like that.

In the past I have used premade templates in programs like printmaster to do something like this but this time I wanted it to be a little more original.

Any ideas?

Maybe I'm missing it. I don't see any that are designed for print use. And FYI yes I did search before I posted.

I would say Illustrator to assemble things, but nothings stopping you from using 20 other prgrams to create the different things you want.

Format is usually chosen depending on the envelops you have, but since they have those nice premate blanco cards A5, i would say: make your card so you will have a A5 format once it is folded.
Practise with folding an A4 (landscape) into A5, so you see what side you want written and which you want to keep blank.
