That looks to me like its been done in a 3D program , and maybe been toutched up in photoshop
Looks to me like theres been some Dodge and Burn ( Tools i love in photoshop )

The images on the right can be done so easy with photoshop with Dodge and Burn tools, but you need to know how use them

Heres Something i did, for my own project using Dodge and Burn
Attached File  Cap1.JPG   24.15K   64 downloads

Heres the link to the full image

If you try this in photoshop, and dont have a 3D program, it would be best looking for some photoshop Dodge and Burn Tutorials

try this site, if you're looking for some good tips and ease of use for PhotoShop... the guy who runs the site actually has his own line of tutorial software and books, so it's absolutely legit. i joined the forum over there a while ago and they have some good stuff:

good luck!
