Hosting Problem

I have this site, i tried to make Feedback Form hosted by etisalat of Dubai.

Last Week I upload the files and tried the feedback form. The Feedback form works perfectly.

But now when i tried send feedback thru website.. no luck.. i didnt received anymore.

What happen? Is their something wrong with the hosting site. Cause I tried to upload the files in different hosting site. The feedback works well..
