ok first start out with a 315x115 image. WHITE OR BLACK BACKGROUND.
Then apply cloud by going to Filter->Render->Clouds.
This is what you should get:

Next:
Reset your colors to black and white by pressing the D key on your keyboard.
Now select some Grundge brushes that appeal to you. If you don't have any go to www.deviantart.com for some brushes.



Now this is the important and fun part. Click randomly once on your image with the brush. DON'T use the same brush it gets really boring. Switch every time you click. Also switch the colors from white to black and black to white every time you click on your image. Just hit X on your keyboard to to this quickly.
Now you should end up with somthing like this or not if you used different brushes. Aslong as you like it!:



Now add some text, I added Power House:


Now right click on your text layer (in the layers box) and click rasterize type. Now click on your text layer in the layer box again. Then do the following:





Now right click the layer that has the text in it in your layer box and click merge down.

Now do the following:





Now your done! Here's the final prouduct:
