I am trying to create a contact form but not working. I have attached the original flash file and the PHP file. It is giving me a lot of errors.

Here are some problems I am seeing so far:

1. On the RESET button at the bottom, it will reset all areas except OTHER check box and the YOUR MESSAGE text box. For some reasons, it does not fit in with the other check boxes. It's siting on it's own.

2. When I click on the SUBMIT button, I am not getting an email back.
