Hi all!

Does anyone know of a website that shows you how to create a contact form with options like First Name, Last Name, Phone Number, check boxes, pull down menu and text box?

I have been searching on the net for tutorials on what I am trying to create but found only basic contact form such as First Name, Last Name, Phone Number and a Text Box for your comments.

It's hard to find tutorials with feautures like Check Boxes, Radial Buttons and Pull Down Menus.
