The +'s aren't so common, most people use hyphens instead. I believe I read somewhere that hyphes are used by Google to seperate words in url's, both domain names and paths.
For an infinite number of levels, change the regular expression to (underscore = level seperator):
^([a-z0-9_]+)$
and use PHP's explode to determine the number of levels, file to included, etc.

$levels = explode('_', $page);
